Please call 708-957-8750 to make an appointment with one of our Physicians or Nurse Practitioners. Horizon triage nurses will discuss the reasons for your appointment. You will be offered an appointment based on the information that you provide to the staff. Please be prepared to tell the staff why you are calling for an appointment.

If you are an existing patient, you may need to schedule a routine follow-up appointment or an acute illness visit. Horizon nurses will work with you to schedule your visit on a timely basis according to the information you provide to them. Acute care appointments are usually available within 24 hours. Routine care appointments are usually available within 10 business days.

Patients new to Horizon can schedule appointments to meet with one of the physicians or nurse practitioners. Your first visit will be a history and physical, an assessment of your health status and a plan of care to address your healthcare needs. If you are calling to schedule a wellness visit, please tell the staff when you call to make your appointment.

If you are a new patient, you will be asked to complete a patient registration form, the Consent for Release and Communications Form and a history and physical form. You will be asked to review the Horizon financial policy and privacy policy. You can access these Forms here and complete them prior to your visit.

On the day of your appointment, please arrive 15 minutes early with your current insurance card, a photo ID, and complete medication list. Co-Payments are expected at the time of your visit as required by your insurance company.

Patient Calls to the Office

When you call the Horizon office, the operator will answer your call or you may be directed to leave a message in the voicemail system. If you are directed to a voice mailbox, please leave a detailed message that includes your full name, the reason for your call, a phone number where you can be reached, and your date of birth. You can leave a message to request an appointment, to request a medication refill, to request a referral for testing, to request your test results, to ask your provider a specific question or to ask a general question. Horizon continuously monitors the messages left on voicemail during office hours. A staff member, who can assist you with your request, will return your call as soon as possible. Please note, voicemail messages left after hours or on the weekends will not be answered until the next business day.

In the case of an emergency, please call 911 or visit your nearest emergency room.

Medication Refills

If you are requesting a prescription refill, please contact your pharmacy where the original prescription is located. The pharmacy will electronically send Horizon your specific request for your Horizon provider to approve.

Please give our office 2 business days to refill your prescriptions. Refill requests are done Monday through Friday, so please make sure you have enough medication and allow adequate time for your refill request to be processed.

If you are requesting a refill of an antibiotic, please contact the office so that the triage nurses can obtain the necessary information for the physician to determine if the refill can be given or if an office visit is needed. Please notify the staff of any medication allergies at the time of your call.
